Do I qualify for Home Care?
A person qualifies for skilled Home Care services if they have the following medical issues:
- Skilled needs such as nursing, physical therapy, occupational therapy, speech therapy or a medical social worker.
- A person must be homebound to receive Skilled Home Care services. If leaving one's home for an activity is "taxing" to the person, they meet the skilled home care definition of homebound.
- Like a medicine prescription, a doctor's order for a Home Care evaluation is a prescription for you to be evaluated for skilled medical services in your home.
Home Care Specialties
- Wound Care - In addition to excellent Home Care Services, Cornerstone offers exceptional Home Care for Wounds. We have nurses with wound care training from the physicians at a Wound Care Clinic. Our nurses are Wound Vac trained also.
- Masters Prepared Medical Social Worker – We are able to offer more Home Care counseling services to our patients due to the certificates that a Masters Prepared Social Worker holds. Patients needing Home Care services, who also have a diagnosis of depression, may qualify for extended counseling with the Master Prepared Social Worker.
