RN/LPN Field Nurse

  Cornerstone Home Health and Hospice has immediate openings in Greene, Fayette, Madison, Miami, and Shelby counties for experienced RNs & LPNs that are willing to travel. We are seeking licensed nurses that would be responsible for planning, coordinating, providing and documenting care for an assigned patient from the day of admission through the day of discharge.

  Utilizing physician’s orders and professional skills, the Home Care Nurse follows a home health plan that meets the patient’s specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as Cornerstone Home Health & Hospice Corp’s policies and procedures.

Job Requirements:

·Knowledge of current federal, state and agency regulations concerning patient care.

·Evaluates patient’s condition, assists with plan of care, re-evaluations and updates as necessary.

·Obtains all necessary physicians orders and collaborates with physician as patients needs dictate.

·Maintains the requested work week as the case has been assigned.

·Follows up with obtaining lab results and notifying physician of any significant results as needed.

·Valid state driver’s license and reliable vehicle.

·Proof of current automobile insurance.

Regulatory requirements:

·Current state licensure as a RN/LPN

·Experience in one of the following

     1. One year acute/SNF care experience within the last 2 years.

     2. One year community health or public health experience within the last 2 years.

     3. One year outpatient facility experience within the last 2 years.

·Preferably has current

     1. CPR card

     2. Current TB test

·Compensation DOE/EOE
