Cornerstone Home Health and Hospice has an immediate opening for an experienced Hospice Coordinator. We currently service Clark, Champaign, Logan and surrounding counties. We are rapidly growing. This position will be responsible for a designated territory and staff, travel will be involved.
Job Description:
- Provide coordination, case management of patient services.
- Provide appropriate documentation for both patient care and staff performance.
- Hire, train, promote and supervise assigned care staff.
- Assist clinical director as necessary.
- Provide direct patient care as necessary to include case management, Oasis care planning, assigning care staff, and ensuring efficient, effective utilization of existing staffing resources.
- Maintains comprehensive working knowledge of Innovative Senior Care Hospice markets; including government agencies and major payer groups.
- Establishes and maintains positive working relationships with current and potential referral and payer sources.
- Work with physicians, nursing, social worker and chaplain to ensure appropriate care and coordination of services for palliative and end of life patient care.
- Assists in establishing organization volume projections in the annual budget, and in establishing allocations for the marketing department. Monitors allocation of resources according to budgetary limitations.
- Provides leadership in strategic planning including identifying opportunities for additional or improved services to address customer needs
Requirements for employment are:
- Current RN licensure within state of branch location, and successful completion of nursing education from an approved school of nursing
- Must have clinical supervisory experience in related field.
- Requires five (5) years of clinical supervisory experience in related field.
- Three (3) years of hospice experience preferred.
- Requires in depth knowledge of homecare regulations.
- Must be able to independently organize work, provide appropriate case management, cost effective home services, and timely accurate documentation of said care.
- Must possess strong critical thinking and communication (oral and written) skills and be literate in the English language.
- Must be able to use a personal computer and have working knowledge of a variety of computer applications (Word, Excel, Windows, etc)
- Must be CPR certified. Potential for exposure to blood and body fluids, sharps and infectious disease.
- Minimum of twelve hours of continuing education annually.
